The case management team focuses on coordinating patient care across the continuum of medical services to ensure the highest level of care, a collaborative process and a safe discharge. Our involvement begins as soon as the patient is admitted. Before leaving the hospital one of our case management team members will discuss discharge planning with the patient and family members to determine if any services are needed (e.g., home care, a skilled nursing facility, short term rehabilitation, sub acute care, hospice, an adult home or a shelter).
At Maimonides, we understand that patients who face illness and hospitalization often have social/emotional concerns that need to be addressed.
