Maimonides has shown improved delivery of all recommended services to patients with congestive heart failure (CHF).
The improvement opportunities we are currently focusing on are:
- Assure that CHF patients receive education and discharge instructions regarding diet, activity level, medications, follow up appointments, daily weight measurements, and signs and symptoms.
- Assure that drugs categorized as ACE-I (angiotensin converting enzyme) and ARBS (angiotensin receptor blockers) are prescribed. These drugs reduce the action of a hormone that can narrow blood vessels. This helps reduce the pressure in the heart and lower blood pressure.

Discharge Instructions
Appropriate instructions for patients regarding activity level, diet, medications, follow-up appointments, daily weight measurement, and monitoring of symptoms is important in managing heart failure.
Left ventricular Function Assessment
An evaluation of the LVS function checks how the left chamber of the heart is pumping.
ACE-I or ARBs for LVSD
ACE inhibitors and ARBs are medicines used to treat heart attacks, heart failure, or a decreased function of the heart.
Smoking cessation advice/counseling
Smoking is linked to heart failure. Quitting may help improve your condition.
